✼ Breast Augmentation
Breast augmentation is one of the most common procedures in plastic surgery and most common surgery performed by Dr Kahng. An implant is placed beneath the native breast tissue to enhance the volume and shape of the breast. There are multiple surgical approaches/incisions (peri- areola, infra mammary fold, axilla) and type of implants ( saline vs silicone cohesive gel vs anatomical form stable silicone gel). Dr Kahng believes in individualizing and customizing your breast augmentation procedure when you have a complimentary consultation. Dr Kahng prefers the co gel ( cohesive gel)/gummy bear implants to achieve a softer more natural look. The silicone cohesive gel/gummy bear have been FDA approved since 2006.


✼ Asian Breast Augmentation
Breast augmentation in the Asian population is still the most common plastic surgery for the body. Due to cultural and anatomical differences, Asian breast augmentation is performed differently compared to the general population. The axillary or infra mammary fold incision is more commonly done due to the smaller areola size. However, Dr Kahng can still use smaller areola if the patient desires to have that approach with the use of the Keller Funnel device. Also due to the narrower chest diameter, skin tightness, and smaller amounts of breast tissue, silicone gel implants are preferred over saline implants to minimize implant palpability and show (rippling). Also moderate profile and anatomic shaped silicone gel implants are more commonly used.

/ Capsular Contracture……What is a capsule?
When an implant (saline or silicone) is placed, the body considers it as a foreign object and creates a thin layer of scar tissue to conceal it from the rest of the body. When it is normal it is called a capsule. The breast feels natural and soft. However, in some patients, the capsule can develop into a thicker and tighter capsule. When this happens, it is called a capsular contracture. The degree of the capsular contracture can be categorized to mild, moderate to severe (Baker Classification I-IV). The reported incidence is about 6-10% of all breast augmentation. Most are mild and do not alter the breast shape. Corrective surgery is necessary if there is an alteration of the breast shape and/or causing pain. The causes of capsular contracture are unknown but smoking tobacco has been associated with a higher risk. The occurrence of capsular contracture can be seen up to 1 year after surgery. To minimize the risk of capsular contracture, there are different techniques (submuscular placement) and types of implants (smooth vs textured) and using the “no touch” implant delivery device such as the Keller Funnel to further decrease the incidence. For mild capsular contracture, medical treatment with Accolate (a medication for asthma) and RF treatment can be prescribed to soften the breast. For moderate to severe capsular contractures, surgical revision is needed. Dr Kahng uses the latest techniques to help treat and prevent recurrences of capsular contractures with the use of acellular dermal matrix (Alloderm, Strattice) and the “no touch” technique.

/ Procedure time and Recovery after Breast Surgery
Dr Kahng performs all his breast surgeries in his AAAASF certified outpatient surgery center within his office. Breast augmentation typically take one hour and if combined with a breast lift can take about 3 hours depending on the type of breast lift surgery being performed. Patient need to be NPO for 8 hours and need to arrive at the surgery center 1 hour prior and usually stay in the recovery room for an hour prior to being discharged. Patients are restricted from driving for 1 week and 1 month for any strenuous physical activity.


✼ Mastopexy [Breast Lift]
Candidates for this breast procedure are those who have adequate breast tissue but with age, the breast tissues have sagged with gravity and also after having children. Another cause is also due to medical or surgical weight loss. Breast lift is performed to elevated the nipple when it is at the level of the infra mammary fold or below. Patients may have adequate volume of breast tissue or have lost the volume. Depending on the amount of excess skin and volume of breast tissue, patient will require one of the three types of breast lift (donut lift, lollipop lift, anchor lift) with or without an implant.


✼ Reduction Mammaplasty [Breast Reduction]
Breast reduction procedure is performed hen patients have more than adequate breast tissue that cause symptoms of back/neck/shoulder pain, as well as difficulty maintaining personal hygiene and developing rash under the breasts. Patients usually experience difficulty finding adequately fitting bras and state the large breasts interfere with daily physical activities. Breast reduction is very effective in relieving such symptoms. This surgery can be covered by your insurance depending on your plan. Breast reduction not only reduces the volume but also lifts the breast tissue.


✼ Male breast Reduction
Gynecomasty is a condition of over-developed breast tissue and tissue in men. It is commonly associated with aging and male hormonal changes, both young and old due to hormonal imbalances. Now a day, it is also commonly seen after significant medical or surgical weight loss procedures in men. With gynecomastia, the condition not only is seen physically but can affect the patient’s mental state, causing social withdrawal. Depending on the type and amount of different tissues, male breast reduction is done with liposuction and removal of glandular tissue. However, if there is excess skin, either a lollipop or inverted T breast lift maybe need as seen in weight loss patients.
